TC Dance & Co. 2025 - 2026 Registration

I accept responsibility for the total monthly fee regardless of attendance. I understand that schedule adjustments may be necessary if class enrollment is not met. I also understand that schedule adjustments may be made for community conflicts such as CCPA and school sports schedules. Tuition is due at the first class of each month. A $10 late fee will be assessed for any payment not paid by the 15th of each month. I give permission for my child’s image to be used for media promotion. I, the parent or legal guardian of the applicant above, hereby give consent for the student to participate in the TC Dance & Co. program and all activities associated with class unless I advise you in writing. To the best of my knowledge, my child is in good health. I understand that the exercise equipment is off limits to my child and will reinforce this rule to my child. I further release and agree to indemnify and hold harmless TC Powerhouse, its owners and instructors from any liability concerning my child’s involvement in its programs and agree that the use of all facilities are at the risk of the registrant.

$25 (per family) registration fee must be received to confirm your child's/children's place in class.
